| Hospitalization / Medical Insurance | ||||||||||||||||||
| For the benefit of our full time employees, the Corporation offers a hospitalization and medical insurance plan. The cost of the plan is partially paid by the employee by payroll deduction. Coverage begins on the first of the month after the employee has worked his or her regular schedule for 30 calendar days. | ||||||||||||||||||
| Coverage for an eligible employee’s spouse and/or dependent children is also available. Payroll deductions are made to cover the cost of additional spouse and/or dependent insurance coverage purchased by the employee. The employee will receive a certificate/booklet which details the benefits provided shortly after the employee is covered under the plan. | ||||||||||||||||||
| Dental Insurance | ||||||||||||||||||
| For the
benefit of our regular full time employees, the Corporation also offers
a dental insurance plan. Coverage begins on the first of the month after
the employee has worked his or her regular schedule for 30 calendar days.

| Short Term Disability Insurance | ||||||||||||||||||
| All full time employees are eligible to enroll in the Corporation’s Short Term Disability program. This plan is designed to pay a weekly benefit that partially replaces lost income during a short-term illness or injury, such as surgery or maternity leave. | ||||||||||||||||||
| Coverage begins on the first of the month after the employee has worked his or her regular schedule for 30 calendar days. The employee will receive a certificate/booklet which details the benefits provided shortly after the employee is covered under the plan. | ||||||||||||||||||
| Long Term Disability Insurance | ||||||||||||||||||
| All full
time employees are eligible to enroll in the Corporation’s Long Term
Disability program. This plan is designed to pay a monthly benefit that
partially replaces lost income during periods of partial or total disability. Coverage begins on the first of the month after the employee has worked his or her regular schedule for 30 calendar days. The employee will receive a certificate/booklet which details the benefits provided shortly after the employee is covered under the plan. |

| Liability Insurance | ||||||||||||||||||
| Professional liability insurance is maintained by the Corporation and provides coverage for all Physical Therapists, Physical Therapist Assistants, Occupational Therapists, and Certified Athletic Trainers. This is a group policy which provides blanket coverage for the clinic as well as the insured employees. All employees are required to immediately report to the Director of Human Resources any incidents of patient complaints or harm so that the incident can be properly handled by the Corporation. | ||||||||||||||||||
| Life Insurance | ||||||||||||||||||
| The Corporation offers life insurance coverage to eligible employees subject to the terms of the current policies. | ||||||||||||||||||
| Vacation | ||||||||||||||||||
| Exempt full time, non-exempt full time, and regular part time employees who are professionally licensed employees are eligible for vacation. The amount of vacation granted based on number of years of service is as follows: | ||||||||||||||||||

| Regular part time professionally licensed employees may be eligible for such vacation on a prorated basis. Vacation accrual will start on the employee’s first full day of employment. Entitlement to vacation shall be based upon the employee’s most recent date of continuous employment. | ||||||||||||||||||
| Paid Holidays | ||||||||||||||||||
| The Corporation recognizes the following national holidays. All exempt and non-exempt full time employees will receive the following holidays as paid holidays (will be paid after 30 days of employment): | ||||||||||||||||||

| If a Paid Holiday falls on a non-scheduled work day, the Corporation will normally recognize this holiday on another day and holiday pay will be paid accordingly. | ||||||||||||||||||
| Employee Birthdays | ||||||||||||||||||
| The Corporation enjoys celebrating an employee’s special day with the employee. For all full time employees, the following is offered each year on their birthday (after 30 days of employment): | ||||||||||||||||||

| Should an eligible employee’s birthday fall on a non-work day or Paid Holiday, the employee will receive an additional 8 hours of regular pay or take another day off as determined by the Director of Human Resources and Compliance. | ||||||||||||||||||
| Vacation Accumulation Rates (Paid Time Off) | ||||||||||||||||||
| The following table shows how vacation is accumulated according to years worked by pay period. The accrual listed below applies to full time employees. Regular part time professionally licensed employees may accrue vacation time on a prorated basis. | ||||||||||||||||||

| Sick Leave | ||||||||||||||||||
| The Corporation grants all regular and full time employees three days (24 hours) of sick leave each year. Sick leave does not accrue from year to year. If an eligible employee must be absent from work for illness but has used all of the allotted sick days, the employee may use vacation time if available. | ||||||||||||||||||
| At the anniversary date (month and day of hire) each employee will receive a new 24 hours of sick leave. Any remaining sick leave from the prior year will be paid to the employee. | ||||||||||||||||||
| Continuing Education | ||||||||||||||||||
| The Corporation grants all full time professionally licensed employees 3 days (24 hours) of continuing education time from May to May each year. These are paid days and do not accrue from year to year, nor do they carry over from the previous year if they are not used by the employee. Regular part time professionally licensed employees may be eligible for such leave for continuing education on a prorated basis. We offer $500 reimbursement annually toward continuing education courses and expenses for all full time clinicians and athletic trainers. | ||||||||||||||||||
